Responsibilities include: • Making home visits to hospice clients within your assigned geographic territory. • Providing skilled nursing care, including medication administration, wound care, treatments, procedures, and comfort-focused care. • Assessing patients' conditions and communicating changes to the appropriate clinical team members. • Implementing and documenting physician orders accurately and promptly. • Performing patient assessments and helping coordinate individualized plans of care. • Collaborating with the interdisciplinary team to support patients and their families throughout the hospice journey. Qualifications • Current Registered Nurse (RN) license in Vermont and New Hampshire. • At least one year of recent, verifiable clinical experience in medical-surgical nursing or a related area. • Previous home health and/or hospice experience preferred, but not required. • Basic computer skills; electronic medical record (EMR) experience preferred. • Ability to travel to patient homes within the assigned service area. • A compassionate approach to patient care and a commitment to clinical excellence.
