Clinical Case Manager – Behavioral Health (Licensed) 100% Remote | Work from Anywhere in the U.S. | Monday–Friday Business Hours No Weekends | No Holidays Are you a licensed Behavioral Health clinician looking for a rewarding remote opportunity where you can make a meaningful impact on members' lives? Join our team and use your clinical expertise to support individuals with complex behavioral and physical health needs through assessment, care planning, crisis intervention, and care coordination. What You'll Do • Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs • Develop individualized care plans that promote improved health outcomes and overall well-being • Collaborate with providers, care teams, and community resources to coordinate appropriate care • Apply clinical judgment to identify and address risk factors, barriers to care, and complex health needs • Utilize Motivational Interviewing techniques to engage members and encourage long-term behavior change • Provide crisis intervention and ensure timely access to appropriate treatment and support services • Coordinate behavioral health and substance use disorder services across the continuum of care • Leverage technology and clinical systems to document care and manage member interactions WHAT YOU BRING - REQUIRED • Master's Degree in Social Work, Counseling, or a related Behavioral Health field • Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent) • 3+ years of post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar behavioral health settings • 3+ years of experience treating mental health and substance use disorders • 3+ years of experience in crisis intervention and using Motivational Interviewing techniques • Strong technical proficiency and ability to document while engaging members telephonically Preferred Qualifications • Managed Care or Utilization Review experience • Case Management experience • Discharge Planning experience • Experience working with complex populations and multidisciplinary teams Why Join Us? • Fully remote position available anywhere in the United States • Predictable Monday–Friday schedule • No weekends or holidays • Meaningful work supporting members with behavioral health and substance use needs • Collaborative team environment with opportunities to make a lasting impact • Utilize your clinical skills in a dynamic, member-focused care management role If you're a licensed behavioral health professional who is passionate about improving outcomes through care coordination, member engagement, and clinical excellence, we'd love to hear from you.
