Position Summary – Field-Based Case Manager RN Roanoke, VA Join Aetna’s Long-Term Services and Supports (LTSS) team in a dynamic, field-based Case Manager role where you can make a meaningful difference in the lives of members throughout Virginia. This position is ideal for a compassionate Registered Nurse or Behavioral Health Clinician who is passionate about delivering personalized, community-centered care and thrives in a role that blends autonomy, travel, and collaboration. As a Case Manager, you will take the lead in assessing, planning, and coordinating comprehensive care for members with diverse medical and behavioral health needs. Through in-person visits (up to 75% travel), you’ll build strong relationships, gain deeper insights into members’ environments, and create tailored care plans that promote both immediate health improvements and long-term wellness. You will play a critical role in integrating services, addressing barriers to care, and connecting members with appropriate resources across healthcare and community systems. By working closely with interdisciplinary teams, providers, and support networks, you’ll ensure care is proactive, coordinated, and aligned with each member’s unique goals. Mileage is reimbursed per our company expense reimbursement policy • RN with current unrestricted Virginia state licensure required. • 3 years Clinical Experience (Preferably in Medical-Surgical Nursing, Behavioral Health, Long term Care, etc.) • 1+ years Managed Care experience • Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding. Preferred Qualifications • Case Management in an integrated model preferred 5 years clinical practice experience • Effective communication skills, both verbal and written. • Bilingual in English/Spanish • LTSS experience preferred Education Associate’s Degree or Bachelors in Nursing, with active Registered Nurse (RN) licensure required; Bachelor’s Degree in Nursing preferred Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and ot
