Remote RN Case Manager (North Carolina) Work from Home | Monday-Friday Schedule | Great Work-Life Balance Are you a compassionate Registered Nurse looking to leverage your clinical expertise in a remote role? Join our team as an RN Case Manager and help improve health outcomes by guiding members through their healthcare journey. What You'll Do • Conduct telephonic assessments to identify members’ medical, behavioral, and social needs • Develop and coordinate personalized care plans to support overall wellness • Collaborate with healthcare providers and multidisciplinary teams to remove barriers to care • Utilize clinical judgment to address complex health conditions and improve outcomes • Support members through transitions of care and connect them with appropriate resources • Maintain compliance with regulatory and company case management standards What We're Looking For - REQUIRED • Active, unrestricted RN license in North Carolina • Must reside in North Carolina • Minimum 2 years of RN clinical experience • Associate Degree in Nursing • Experience in any clinical setting considered, including: • ICU, NICU, Emergency Department • Med/Surg, Telemetry, OR • Home Health, Hospice • Long-Term Care, Infusion, Orthopedics, and more • Strong communication and member engagement skills Preferred Qualifications • Certified Case Manager (CCM) • BSN • Case Management or Managed Care experience • Discharge Planning experience • Proficiency with Microsoft Office Suite Schedule & Location • 100% Remote (Telework) • Must live in North Carolina • Monday-Friday, 9:00 AM-5:30 PM EST • Occasional evenings, weekends, holidays, and limited travel (up to 10%) Why Join Us? • Remote work flexibility • Opportunity to make a meaningful impact on members' health and well-being • Professional growth in care management • Company-paid support for obtaining additional state RN licenses Apply today and use your nursing expertise to help members achieve healthier outcomes from the comfort of your home.   Work from Home Requirements: • You must have or be able to obtain a direct/hardwired internet connection to a modem/router within 7 feet of your computer and a minimum download speed of 25 mbs download and 3 mbs upload.
