About the role

Territory Coverage: Bergen County, New Jersey. Travel throughout the assigned service area is required. You see the medications on the table, the fall risks, the caregiver dynamics, the food insecurity, the missed follow-ups, and the small clinical changes that can become major health events if no one catches them early. Clover is building a model around that level of visibility. We are seeking a compassionate and driven Nurse Practitioner who is bilingual in Korean and English to join our growing Clover Home Care Team in New Jersey. This hybrid role supports Medicare Advantage members throughout Bergen County, combining in-home visits with telehealth care. Korean and English fluency is essential to building meaningful relationships with Korean-speaking members and caregivers, supporting clear communication, and improving outcomes for medically complex and vulnerable senior populations. What You'll Do • Conduct in-home and telehealth visits for Medicare Advantage members. • Provide care and patient education in Korean and English to support effective communication with members and caregivers. • Perform comprehensive assessments, medication reviews, and chronic disease management. • Develop and implement individualized care plans that address both clinical and social determinants of health. • Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence. • Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic, member-centered care. • Leverage Clover Assistant and EMR technology to support proactive care management and close gaps in care. • Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations. • Educate members and caregivers on disease management, preventive care, and treatment plans. Qualifications • Active Nurse Practitioner (NP) license in New Jersey or ability to obtain licensure. • National board certification as a Nurse Practitioner. • Active DEA and New Jersey CDS registration, or ability to obtain. • Fluency in Korean and English, with the ability to conduct clinical assessments, explain treatment plans, and provide patient and caregiver education in both languages. • Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings. • Strong clinical assessment and chronic disease management skills. • Passion for serving vulnerable senior populations. • Comfortable utilizing technology and telehealth platforms to enhance patient care. • Excellent communication and collaboration skills. • Preferred Qualifications • Home care, home health, home-based primary care, palliative care, geriatrics, or population health experience. • Experience caring for Medicare Advantage members. • Prior experience in value-based care models. • Experience managing medically complex and high-risk patient populations. • Experience providing culturally responsive care to Korean-speaking patients and families.
