Case Manager (RN) – Field & Telephonic Location: Dallas Service Delivery Area Remote with local travel of up to 75% of the time Position Summary Nurse Case Manager (RN) for a work-from-home role that requires frequent local travel for in-home member visits and occasional visits to nursing or medical facilities within the Dallas Service Delivery Area. In this role, you will deliver high-quality case management services through a combination of telephonic and face-to-face engagement, helping members navigate their healthcare needs and achieve improved health outcomes. Required Qualifications • Licensed Registered Nurse in Texas • 3 or more years of Clinical Nursing Experience • 1 or mor years case management experience • Must reside in Dallas TX Service Delivery Area • Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy Preferred Qualifications • Experience in a Managed Care Organization (MCO) • STAR Kids program experience • Bilingual (English/Spanish) preferred Education: • Associates Degree in Nursing Required • Bachelor's in Science & Nursing Preferred Work from Home Requirements: • You must have or be able to obtain a direct/hardwired internet connection to a modem/router within 7 feet of your computer and a minimum download speed of 25 mbs download and 3 mbs upload.
